Died July 22, 1914 in Omaha, Douglas County, Nebraska by Dr. Levi "Al" Lemonds of Bartlett and well known in the southwest part of Mills County, was almost instantly killed at Omaha last night when he jumped or fell from the third story window of the Lyons Hotel at 113 North 13th Street. Note: This is the only stone in this cemetery located at 66497 Pease Road, Glenwood, Mills County, Iowa
